Finding the sign-up information at the Michael J. Fox Foundation web site is not so easy. Here is the press release that tells how the Foundation is participating in the program:

The above article states a cutoff date of March 22, 2009 for receiving the discount code, but I applied over a month past that date. The discount code arrived by email a couple weeks later!
